WALK IN'S:  For any patient who shows up to the office and would like to be seen right away, we can often accomodate this.  If we can't accomdate you right away, we should be able to give you the next available appointment, most often by the next day.  Please bring in your insurance information, MRI scans, etc. (if available), fill out our paper work in the office.  If you have a HMO, then you will need a referral.  Medicare, private insurances, cash patients are all welcome for a walk in appointments.

REGULAR Office Appointment, you may contact our office staff and obtain a consultation date.  In order to expedite a new consultation, this can easily be done and in doing so you may be seen immediately.  To expedite the process you will need to fill out the patient information sheets (available as the New Patient Packet on line or in the office), then bring in,  mail, fax or e-mail the information to our office.  It is helpful, but not necessary, to have any pertinent scans or x-rays of the involved area brought to our office staff so that we will be able to review those studies immediately or very soon after the information is brought to the office.  You may find the necessary forms to complete on this web site under the in a PDF format New Patient Packet.pdf.  You may download this information, fill it out and mail it or fax, or perhaps scanned and have it e-mailed to our office.  You will be given a appointment which can be expedited if you let us know you would like to be seen ASAP.  There is an Arbitration Agreement document which we request you to review, print and sign.  If you decide to not sign the arbitration agreement we will still be happy to see you in consultation.

HMO Patient's,
You need to make sure you have the referral send to our office in advance of the appointment.  If you do not have the referral, then you will need to obtain this form the referring physician (your primary care doctor or referring specialist), perhaps by calling the referring doctor, etc.. Please also fill out the "New Patient Packet" information.

PPO and/or Medicare Patients,
You do not need a referral to be seen by a one of the neurosurgeons. You may want to check with your particular insurance to ensure that our neurosurgeons are providers for your PPO plan.  Otherwise, proceed with the New Patient Packet information and contact our office. 

Workman's Compensation
The neurosurgeons need to review information about your case prior to setting up an appointment.  It is important to bring your films and any written information you may have regarding your particular case.  Once this information is review, our neurosurgeons will determine if they would be able or willing to get involved with your Workman's Compensation claim.

Non-contracted Insurance Groups (ProMed, PrimeCare Inland Valley, Kaiser)

We are happy to schedule appointments for you.  However, your out of plan insurance group will not cover the expenses for a consultation with LDR.  Therefore, you would be required to pay in cash or a credit card for these appointments.  If you are willing to do this, you may fill out the New Patient Packet and forward the packet along with your films for review to our office.  You can contract our office staff to get an idea of what a new consultation would cost.  We do request for a $100 commitment deposited in order to set up appointments with patients in this category.  We can see you right away if you are intested in seeing us for a second opinion.

For MISSED consultations a consultation that is not canceled within 24 hours you will be billed for $100 for not showing up or for not calling us with canceled consultations.  For those patients who pay the commitment deposited of $100 who do not show up for the appointment the deposit will be kept by LDR.  If you know you cannot make the consultation time and date, then contact us as soon as possible so we can change or cancel your consultation time and refund your deposit.

MRI scans, etc. we recommend that you personally obtained the films (either the films or on a CD) and personally bring them to our office.   The facilities were these films were done will not mail them, or if they do, it will take a long time to receive them and we may not receive the films in time for your consultation.

Reminder calls, e-mails for your consultation will be made by one of the office staff. If we do not get confirmation of your intent to show up for the consultation, then your consultation time may be given away as we generally will set aside 60 minutes for a consultation.   We would appreciate it if you confirm with our office at least 24 hours prior to your consultation time.   This can be done via e-mail to Ms. Tricia Cordero or Ms. Elena Salcedo (see "OFFICE" for e-mail addresses).  Messages can be left on our answering machine, but then we may not get to those calls in time for the following morning.

Please be aware that occasionally the appointment scheduled may be disrupted by our needs to attend to a seriously ill patient or emergency surgery.  Your understanding in these situations is appreciated.

If you have any questions about your insurance coverage, including Medicare, please ask a member of our office staff.

Requests for prescription refills are processed during normal office hours (9:00 a.m. to 5:00 p.m., Monday through Friday).  We do not refill prescriptions on weekends.
